Multiple Drug-resistant Lumbosacral Tuberculous Spondylitis in a Foreign-born Immunocompetent Young Adult- a Case Report

چکیده
Spinal tuberculosis (TB) is a dangerous form of skeletal TB because of its ability to cause bone destruction, deformity, and paraplegia. The emergence of multiple drug-resistant strains (MDR-TB) should be suspected if treatment failure occurs, either medically or surgically, because its global incidence is increasing. Patients with MDR-TB are more difficult to treat, remain infectious for longer period of time, and may pose a J. Orthop Surg Taiwan 22: , 2005 9

Advanced multifocal tuberculous spondylitis without disk involvement and with multidrug-resistant bacilli.
In this brief report, we describe a 23-year-old immunocompetent Nigerian patient with extensive multifocal tuberculous spondylitis without disk involvement. Cultures of sputum samples and biopsy samples from the L4 vertebra were positive for tuberculosis; drug-susceptibility testing of the isolates revealed multidrug resistance. Treatment with second-line drugs resulted in an excellent interim ...

Disseminated tuberculosis (TB) refers to involvement of two or more non-contiguous sites. It is a common mode of presentation of tuberculosis in patients both with and without HIV/AIDS in India. However, the presence of primary drug resistance in disseminated tuberculosis involving only the extrapulmonary sites in an immunocompetent adult is rare. Extrapulmonary TB accounts for approximately 15...
متن کاملClinical Improvement of Non-Surgical Management of Tuberculous Spondylitis: A Case Report
Background: Tuberculosis is the second most common fatal infectious disease after Acquired Immunodeficiency Syndrome (AIDS) in the world. The spine is involved in 50% of osteoarticular tuberculosis cases. Tuberculous Spondylitis (TS) is the most dangerous form of osteoarticular tuberculosis, because of its ability to destroy the vertebral body with subsequent permanent kyphosis and neurological...
متن کاملTh1/Th2 Imbalance and Elevated PD-L1 in Pleural Effusion Predict the Risk of Multi-Drug Resistant Tuberculous Pleuritis
Background: Patient immune status might be indicative of the variance in bacterial genetics in drug-resistant tuberculous pleuritis and could be used for predicting the risk of multi-drug resistant tuberculous pleuritis (MDR-TB). Objective: To determine the significance of Th2/Th1 ratio and concentration of PD-L1 in the pleural effusions for prediction of MDR-T...
متن کاملTuberculous lumbar spinal epidural abscess in a young adult (case report)
INTRODUCTION Spinal Epidural abscess (SEA) is an uncommon pathology that needs an urgent intervention to decompress the pressure on the spinal epidural sac, cord, and roots. The authors report a rare case of a young adult with lumbar spinal epidural tuberculous abscess occupying the spinal canal from L2-L5 vertebrae with extesion to the posterior paraspinal muscles and presenting with bilateral...
